In the event that this Guidance is used by persons other than Oxford University Hospitals NHS Foundation Trust, including health professionals working within other hospitals or elsewhere:
- The user is expected to apply their own hospital's or institution's guidance and policies; and

Scope
The Guidance is designed as a support and reference tool for health professionals working within Oxford University Hospitals NHS Foundation Trust and it is only intended for use as a support and reference tool. In particular, users must:
- Be aware that the Guidance does not take into account the individual circumstances of a patient and may not contain all the information required. It must not be used as the sole basis for prescribing any drugs or for the care of any patient. Updates may be released periodically and it the user’s responsibility to ensure that you are using the most up to date version available.
- Be aware that other hospitals have their own guidance documents in relation to the subject matter of this Guidance. Such guidance documents should be used by persons working in hospitals other than Oxford University Hospitals NHS Foundation Trust and, in these circumstances, the guidance contained in such documents takes precedence to any information supplied within this Guidance.
