Ceftolozane with Tazobactam (Zerbaxa®)

General Information

Do NOT use if patient has a history of severe penicillin allergy. If in doubt, discuss with Micro/ID.

Restricted formulary antimicrobial: Must be recommended/prescribed by a Micro/ID consultant or registrar.

AWaRe antibiotic classification: 'Reserve'. All indications to be discussed with Micro/ID.

Renal impairment

on haemodialysis days, the dose should be administered at the earliest possible time following completion of haemodialysis

Hepatic impairment

No dosage adjustment required

Pregnancy

Use only if benefits outweighs the risk.

Breastfeeding

Discuss with pharmacy.

Ceftolozane and Tazobactam 1.5g vial contains 1g ceftolozane and 0.5g tazobactam powder for reconstitution.
Reconstituted solution with sodium chloride 0.9% contains 11.5mmol of sodium. This should be considered when administering to patients who are on a controlled sodium diet.
