Renal and hepatic impairment

Renal impairment

Creatinine clearance (mL/min) Dose
20-50 28mg/kg every 8 hours
10-20 15mg/kg every 8 hours
less than 10

6mg/kg every 8 hours

HD/ HDF/High Flux/PD

Dose as in creatinine clearance less than 10ml/min.

Dialysed; give after dialysis.

Note: dose ranges use creatinine clearance, rather than eGFR.

 

Hepatic impairment
No dose adjustment required in hepatic impairment.

Additional information

  • Risk of renal toxicity – ensure adequate hydration and monitor renal function closely.
  • Monitor serum calcium and magnesium
  • Foscarnet comes as a 24mg/ml solution – round dose to nearest measurable quantity
