Gentamicin - Infective Endocarditis Monitoring

This guide relates to synergistic gentamicin 3mg/kg once daily dosing for infective endocarditis. For other gentamicin monitoring see HERE

Auditory function may be affected. Please refer to Audiometry and intravenous aminoglycosides. 

Monitoring

When to take levels:

  • Creatinine clearance 30mL/min and over: Check trough levels prior to day 3 administration.
  • Creatinine clearance less than 30mL/min: Check trough levels prior to day 2 administration and wait for level. Do not redose until the level is less than 1mg/L

 

Levels and action to take: 

Target level: Trough level less than 1mg/L

Discuss levels over 1mg/L with Micro/ID or pharmacy as dose adjustments may be required. 

 

Frequency of monitoring:

  • For patient with stable renal function and trough level less than 1mg/L: Three times a week. 
  • For patient with creatine clearance less than 30ml/min or unstable renal function or trough level more than 1mg/L: Daily level
