Renal and hepatic impairment

Renal impairment

eGFR (mL/min/1.73m2) Dose
More than 10 Dose as in normal renal function
Less than 10 200-300mg OD
HD/HDF/High Flux/PD

Dose as in eGFR less than 10ml/min/1.73m2.

Dialysed; give after dialysis

  • Risk of ototoxicity and peripheral neuropathy enhanced in renal impairment. Ensure pyridoxine is prescribed.

 

Hepatic impairment

  • Use with caution in hepatic impairment.
  • Monitor liver function regularly, particularly in first 2 months of treatment.

Pregnancy and breastfeeding

Pregnancy

Not known to be harmful in pregnancy; ensure prophylactic pyridoxine is prescribed.

Breastfeeding

Discuss with pharmacy. Theoretical risk of convulsions and neuropathy in breastfed infant; prophylactic pyridoxine advisable in mother

Additional information

Risk of severe hepatitis. Advise all patients and/or their carers to recognise signs of liver disorder.
