Standard dose

 

 

oral Trimethoprim

Treatment dose 200mg po bd
Prophylaxis dose

100mg po on

Alternatively 200 mg for 1 dose, dose to be taken following exposure to a trigger.  

 

Obesity

No dose adjustment required in obesity.

Renal and hepatic impairment

Renal impairment

eGFR (mL/min/1.73m2)

Dose as in normal renal function

Less than 15

50 - 100% of normal dose

HD/HDF/High Flux/PD

Dose as in eGFR less than 15mL/min/1.73m2

HD/HDF/High Flux: Dialysed; give after dialysis

Hepatic impairment 

Avoid in severe hepatic impairment.

Pregnancy and breastfeeding

Pregnancy: Contraindicated in pregnancy; teratogenic risk in the first trimester. 

Breastfeeding: Present in breastmilk – short term use not known to be harmful
