Renal impairment

For CMV prophylaxis only (post solid organ transplant):

Creatinine clearance (ml/min)

Oral dose

Greater than 30

1g TDS

10-30

1g BD

Less than 10 or HD

1g OD (give after dialysis)

Note: dose ranges use creatinine clearance, rather than eGFR.

Hepatic impairment

Use high doses with caution in patients with severe hepatic disease.

Discuss with pharmacy.

Additional information

  • Ensure patients have adequate fluid intake whilst on valaciclovir.
  • Valaciclovir is only available by the oral route (as a tablet only).
