Adult
Pyelonephritis/ upper UTI (Transplanted kidney)
- Pyelonephritis (upper UTI) consists of fever, loin pain and urinary symptoms (WITH positive urinalysis)
- Always take blood and urine cultures, ideally before commencement of antibiotics
- Treat according to sensitivity test results
- Treat for 7 days (review IV daily)

Including those with penicillin allergy (non-severe)
ceftazidime 1g iv tds
Alternative
For penicillin allergy (severe)
ciprofloxacin* 500mg po bd (400mg bd iv if unable to swallow)
For MRSA positive patients: ADD vancomycin iv
