Pyelonephritis/ upper UTI (Transplanted kidney)

Pyelonephritis (upper UTI) consists of fever, loin pain and urinary symptoms (WITH positive urinalysis)
Always take blood and urine cultures, ideally before commencement of antibiotics
Treat according to sensitivity test results
Treat for 7 days (review IV daily)

Including those with penicillin allergy (non-severe)
ceftazidime 1g iv tds

For penicillin allergy (severe)
ciprofloxacin* 500mg po bd (400mg bd iv if unable to swallow)

For MRSA positive patients: ADD vancomycin iv
