Epididymo-orchitis

Warning
  • A complete sexual history is imperative 
  • If sexually transmitted infection is considered, contact Oxford Sexual Health Service (01865 231231) department for notification, screening and contact tracing advice.
    • Send urine or urethral swabs, rectal swabs and throat swabs for screening of chlamydia and Gonorrhoea. Use EPR ‘PID/STD/Gonorrhoea/Chlamydia/Trichomonas PCR genital samples’ Careset
    • Screen for HIV and syphilis (inform patient)
  • For in-patients, consult Micro/ID
  • For patient with penicillin allergy label consider penicillin allergy assessment and delabelling

Treat for 10-14 days

Preferred

Probably due to Chlamydia/Gonorrhoea: 

ceftriaxone 1g IM single dose PLUS doxycycline 100mg po bd for 14 days

 

Most probably due to enteric organisms (usually over 35 years old):

co-trimoxazole 960mg po bd for 10 days

OR

cefalexin 1g po tds for 10 days 

 

When mycoplasma genitalium is suspected: discuss with Sexual Health
