Minor skin operations

Preferred: flucloxacillin 500mg po 

For penicillin allergy (non-severe, severe) or for MRSA positive patients: doxycycline 200mg po

Plastic surgery - clean surgery no prosthetic material

Preferred: flucloxacillin 2g iv

For penicillin allergy (non-severe): cefazolin iv (see dosing table below)

  cefazolin iv dose 
Weight under 50kg 1g
Weight 50kg to less than 120kg 2g
Weight 120kg or more 3g
Creatinine Clearance less than 10ml/min or patient receiving Haemodialysis 1g

For penicillin allergy (severe) or for MRSA positive patients: gentamicin 3mg/kg iv

Plastic surgery - contaminated/ infected or involving mucosal surfaces or with prostheses

Including:

  • hypospadias
  • cleft palate
  • cranio-facial surgery
  • ulcerated skin lesions (inc. pressure ulcers)
  • tissue expanders and implants
  • axillary/ groin dissection
  • hand trauma
  • breast surgery inc. prostheses

Preferred (including for penicillin allergy (non-severe)): cefazolin iv (see dosing table below)

Weight 120kg or more 3g

For MRSA positive patients: add teicoplanin 800mg iv

Burns, skin grafting

No prophylaxis needed. Treatment courses appropriate when infection present.

Leech Therapy

ciprofloxacin 500mg bd po until leeches off.
