EPR Communicate Message ~R-ED_H@H to Hospital at Home (Central) 

Hospital at home is an alterative to medical admission and is a hospital outreach service for delivering hospital level care in the community. Please consider GP follow up, Rapid Access Geratology Clinic, or encouraging patient to self-referral to SPA for patients that would not otherwise meet criteria for inpatient management.  

The hospital at home team includes doctors, nurses and pharmacists and can review patients daily. Diagnostics include POC bloods, POC ultrasound/echo and access to specialist advise. We can deliver IV therapy (diuretics, antibiotics) oxygen and can perform med reg level therapeutic procedures (drains).   

There are no strict inclusion criteria and H@H accept referrals that would otherwise be admitted to EAU – we commonly look after decompensated frailty syndrome, bacteraemia, decompensated heart failure.  We can put in next day carers and help facilitate therapy reviews within 48h.  

Exclusion Criteria:  Patient does not Consent, Unsafe environment / social setup, Risk of violence to staff / lone workers (please discuss if unsure)

DAY TIME (8am-8pm) 

1) Call H@H on ext 27799, bleep 8118 or 07887631924.   

2) Then send the Communicate message.   

NIGHT TIME (8pm-8am)  

1) Send the Communicate message.  

2) H@H team will pick it up at 8am and take over your patient's care.
