Posaconazole tablets and oral suspension are NOT interchangeable. Seek advice from Pharmacy if changing between formulations.

NB: Please be aware that OUH has posaconazole 40mg/mL suspension on formulary - the use of this liquid in paediatric patients is off-label. Another posaconazole suspension with a different strength is available in the UK but not used at OUH. The two oral suspensions are not interchangeable – discuss with Pharmacy.
