For post-op doses: Two doses of co-amoxiclav 30mg/kg (max 1.2g) iv every 8 hours (every 12 hours for patients 1-2 months old) may be given post-op for prophylaxis at clinician discretion.

For penicillin allergy (non-severe)

For penicillin allergy (severe) or MRSA positive
teicoplanin 10mg/kg iv (Max 800mg)
