Augmentation cystoplasty (involving small bowel)

Link to General Principles of Surgical Prophylaxis including redosing table and renal dosing for cefazolin.

Continue oral antibiotics for 5 days post-operatively.

Preferred

metronidazole 30mg/kg iv (Max 500mg)

AND

cefazolin iv see dosing table below

Weight less than 120kg 30mg/kg (maximum 2g)
Weight 120kg or more 3g

 

Post op doses: co-amoxiclav po (see dosing table) for 5 days post-op

0.25mL/kg of 125/31mg/5mL TDS

1-5 years

5mL of 125/31mg/5mL TDS

6-11 years

5mL of 250/62mg/5mL TDS

12-17 years

250/62mg TDS (½ x 625mg tablet TDS

OR 5mL of 250/62mg/5mL TDS)

Alternative

For penicillin allergy (non-severe)

metronidazole 30mg/kg iv (Max 500mg)

AND

cefazolin iv see dosing table below

Post op doses: trimethoprim 4mg/kg (Max 200mg) po BD for 5 days post-op

 

For penicillin allergy (severe) or MRSA positive

teicoplanin 10mg/kg iv (Max 800mg)

AND gentamicin 2.5mg/kg iv (Max 480mg)

AND metronidazole 30mg/kg iv (Max 500mg)

Post op doses: trimethoprim 4mg/kg (Max 200mg) po BD for 5 days post-op
