Continue trimethoprim post-operatively while catheterised.
Preferred including for penicillin allergy (non-severe)

If urinary tract abnormality, add gentamicin 2.5mg/kg iv (Max 480mg) stat dose
Post op: trimethoprim 2mg/kg po nocte (Max 100mg) until catheter is removed
Alternative
For penicillin allergy (severe) or MRSA positive
teicoplanin 10mg/kg iv (Max 800mg)
AND gentamicin 2.5mg/kg iv (Max 480mg)
Post op: trimethoprim 2mg/kg po nocte (Max 100mg) until catheter is removed
