Buprenorphine Patches
There are 2 different brands of Buprenorphine patch, so it is important to clarify with the patient which one they use. BuTrans® patches last for 7 days, while Transtec® patches are changed every 4 days.
These patches should not be started in the Trust, but may be continued if the patient takes them long-term.
BuTrans® buprenorphine patch
Patch duration: 7 days so prescribe once a week
Onset of effect: 12-24 hours before full analgesic effect of patch occurs
Off set of action: 12-24 hours before analgesic effect wears off completely
Prescribe short acting opioid for breakthrough pain
Always co-prescribe Naloxone (See Opioid Toxicity).
Transtec® buprenorphine patch
Patch duration: 96 hours so prescribe every 4 days
Onset of effect: 12-24 hours before full analgesic effect of patch occurs
Off set of action: 12-24 hours before analgesic effect wears off completely
Prescribe short acting opioid for breakthrough pain
Always co-prescribe Naloxone (See Opioid Toxicity).
*If a background infusion on a Morphine PCA is being used to replace a patch, start at a lower dose (for example, 50% of calculated) then review pain management later and increase if needed.
