Antibiotic Regimens:

Route	Antibiotic and Dosage	Notes
Oral (Preferred)	amoxicillin 500mg po TDS AND nitrofurantoin 50mg po QDS	Alternative to nitrofurantoin: trimethoprim 200mg po BD Oxford Health will use nitrofurantoin M/R 100mg BD (not on formulary at OUH). See Nitrofurantoin MHRA drug safety updates
Oral (Preferred)	co-trimoxazole 960mg po BD
IV (Aim for rapid oral switch)	amoxicillin 1g iv BD AND gentamicin 5mg/kg iv STAT	See gentamicin monograph for dosing (including renal dosing)
	doxycycline 100mg PO BD AND gentamicin 5mg/kg iv STAT	Doxycycline is oral. See gentamicin monograph for dosing (including renal dosing)
	co-trimoxazole 960mg iv BD	Pause ACE inhibitors/Angiotensin receptor blockers or Potassium sparing diuretics

If intra-abdominal source suspected: ADD metronidazole 400mg po TDS to the above regimens (except co-amoxiclav which already covers anaerobes).
