UTI with sepsis/ severe sepsis (Urosepsis)

Warning

Longer duration may be required for complicated urological cases, including those with ureteric stents (7-14 days).

Treat according to sensitivity test results 

Treat for 5-7 days (review iv daily)

Preferred

amoxicillin 1g iv tds AND gentamicin 5mg/kg iv. (Gentamicin is for a maximum of 3 days). Take a gentamicin level at 6-14 hours after first dose to calculate dosing interval, usually 24 hourly, 36 hourly or 48 hourly. See gentamicin monograph for dosing (including renal dosing) and monitoring. 

Alternative

For penicillin allergy (non-severe and severe)

co-trimoxazole 960 mg po bd (iv if NBM)

OR

gentamicin 5mg/kg iv (Gentamicin is for a maximum of 3 days). Take a gentamicin level at 6-14 hours after first dose to calculate dosing interval, usually 24 hourly, 36 hourly or 48 hourly. See gentamicin monograph for dosing (including renal dosing) and monitoring.

Oral follow on therapy options include:

co-trimoxazole 960mg po bd

OR

amoxicillin 500mg po tds

OR

cefalexin 1g po tds
